Continuum Home Health, Inc. is a non-profit Medicare-certified licensed agency that provides comprehensive home health services to clients with behavioral, developmental, substance abuse or medical needs. The agency began in 2001 as a sister agency to Continuum of Care, an agency that has been a leader in providing residential case management and community-based alternatives to psychiatric inpatient care since 1966.

The agency ranks among prominent home health providers in the greater New Haven area and is recognized by mental health physicians and clinicians as an agency that is highly specialized in home health care for the behavioral health, developmental and substance abuse population.

Continuum Home Health’s distinct Healthy Mind, Healthy Living© program gives referral sources security in knowing their clients will receive comprehensive home care and support services.
Healthy Mind, Healthy Living ® is a unique home care program that provides a specialized integrated approach to clients with behavioral health, developmental or substance abuse needs. The program combines skilled care management with day to day case management.

Upon admission, Continuum Home Health immediately partners with the client’s case management agency. If the client does not have a case management agency, Continuum Home Health facilitates this service until the patient is referred and accepted by one. Case management may include making medical appointments for clients with their primary care clinicians, while care management includes provision of skilled nursing, home health aide, physical rehabilitation, and social work services.

Clients receive assistance with managing their day to day life activities, as well as skilled assessment and management of their clinical needs. This often includes medication administration. The client’s primary care nurse works in concert with the physician to develop a client-specific plan of care.

Each client is cared for and managed by an interdisciplinary team. Led by the client’s Primary Care Nurse, the team communicates regularly to ensure optimum coordination of services so the client’s specific needs are met.
