The fundamental mission of Continuum of Care, Inc. is to provide comprehensive community-based residential and support services that enable persons with psychiatric and developmental disabilities to live as fully and independently in the community as possible.

Continuum of Care, Inc. is a non-profit, 501 (c)(3) organization that has been providing housing and residential support services to persons with psychiatric disabilities since 1966 when it opened one of the first transitional “halfway houses” in the country. Continuum of Care Inc. changed its name in 1981 (formerly New Haven Halfway House, Inc.) to reflect its ever-changing goals and philosophy.

Continuum of Care, Inc. is an integral part of the Community Services Network (CSN), a collaboration of many service providers in the Greater New Haven Area who provide services aimed at fulfilling the mission of the State Department of Mental Health and Addiction Services. Continuum of Care Inc. is also an active participant in the Citywide New Haven Continuum of Care (NHCOC) collaboration.

Over the years, Continuum of Care established new partnerships and collaborations with the US Department of Housing and Urban Development (HUD), Connecticut Department of Developmental Services (DDS), the Housing Authority of New Haven (HANH), and the Department of Social Services (DSS).

  • Acquisition of independent living skills
  • Respecting and fostering client choice, and
  • Individualizing goals objectives based on the client's preference

Within the programs of Continuum of Care, the keystone to coordinated and continuous community-based services is the designation of clear responsibilities for client care. Upon admission, Continuum of Care immediately coordinates and partners with the client’s permission any and all community providers including but not limited to clinical, homecare, vocational, psychosocial, behavioral, and other support providers. Where appropriate, family and significant others will be encouraged to be involved.

Continuum of Care also coordinates other essential natural supports such as employment, education, or volunteer opportunities. In addition to professional staff, Continuum of Care provides an experiential setting for students, interns, and volunteers who provide a wealth of expertise for strengthening clients’ community skills.

Continuum of Care is dedicated to providing comprehensive residential case management, counseling, crisis, respite and housing support services to adults challenged by developmental disability, psychiatric disability, homelessness, substance abuse treatment needs, and other specialized needs, with the goal of full and productive integration with the community. The following is a listing of residential services provided through Continuum of Care:
  • Case management
  • Assistance with Activities of Daily Living (ADL)
  • Rehabilitative life skill training
  • Budgeting and money management
  • Pre-vocational and employment training
  • Referrals for medical appointments, psychiatric counseling, addiction treatment, and dental/ancillary services
  • Transportation and travel training for use of public transportation
  • Assistance with compliance in legal obligations including probation
  • 24-hour crisis intervention with access to respite bed and services
