Ms. Walker has been the lead executive at Continuum of Care Inc. for 30 years. During those 30 years she has directed the development of the Agency into an integrated provider of residential services for persons with Mental Illness. These services encompass a wide array of programs designed to meet the acute needs of the individuals, as well as, provide residential support programs and case management services. This growth is evidenced by the development of 30 statewide residences operated by the Agency with more than 500 persons employed to service individuals with disabilities.
During the most recent decade, Ms Walker led the Agency in developing programs for dually diagnosed clients, persons suffering from mental illness and developmental disabilities. Under her direction, specialized services for persons with autism have become a major focus for the Agency.
Recognizing the need for synergy between the behavioral and medical needs of individuals, Ms. Walker established Continuum Home Health Inc. in 2001. This created a coordinated model of residential services supported by nursing care that has become a model for the Greater New Haven behavioral health community. Providing home care to residents of the New Haven Housing Authority, residential support programs throughout the city and private homes has enabled many individuals with disabilities to remain independent and at home.
Ms. Walker holds a Master of Social Work degree from Fordham University. She began her career in the human service field working with developmentally disabled individuals in both the residential and vocational setting. Ms. Walker serves on the Community Services Network Leadership Group of the Connecticut Mental Health Center and served for many years as a member of the Board of Directors of the Regional Mental Health Board servicing the Department of Mental Health and Addiction Services. She is also a member of the Board of Directors of the West River Neighborhood Services Corporation.
Martin Morrissey, MHA, CPA, FACHE

Mr. Morrissey has been the Chief Financial Officer and Vice President of Human Resources for all Continuum Companies for the past nine years. This role encompasses the oversight of 5 corporations, 30 properties, and 560 employees.
Mr. Morrissey has a wealth of experience in business and healthcare management. Prior to joining Continuum, he served as Vice President of Finance, Executive Vice President and Chief Operating Officer, and Interim Chief Executive Officer of Saint Mary's Hospital and its affiliated corporations in Waterbury, CT. He is board certified in managing healthcare organizations by the American College of Healthcare Executives (FACHE), a Certified Public Accountant, and he holds a Master's Degree in Healthcare Administration.
Mr. Morrissey has served in leadership roles on the governing bodies of many pertinent healthcare providers including an acute care hospital, a home healthcare agency, a surgery center, a radiology network, a captive insurance company, and many social service providers. He brings that experience to Continuum along with a distinguished career in managing banking, insurance, and regulatory relationships.
James Farrales, LCSW, ACSW

Mr. Farrales has been serving on the executive management team of Continuum of Care for the last 12 years. Key to his role for Continuum is the clinical and programmatic support of the staff for all the mental health programs of the Agency. Mr. Farrales has successfully secured many grants that have led to new programs that have contributed to the Agency's service expansion. He has also developed Continuum's quality improvement program, Medicaid reimbursed Rehab Options (MRO) initiative, Corporate/HIPAA Compliance, and staff development and training programs.
Continuum, under Mr. Farrales' supervision, provides an ideal field placement for many area universities and colleges. Many bachelors and postgraduate students intern in many of the Agency's programs, as they fulfill their academic programs. These interns contribute a great deal of professional expertise as they enhance the quality of our services. Jim has a long history of working collaboratively within a network of agencies within the behavioral health sector.
Mr. Farrales holds a Master's Degree in Social Work and a Bachelor's in Public Health. He is a Licensed Clinical Social Worker and a member of the National Academy of Certified Social Workers. He serves as co-chair of the Steering Committee of the Greater New Haven Regional Alliance to End Homelessness. He also serves as a board member for the Emergency Food and Shelter Program of the United Way of Greater New Haven, as well as the Advisory Board of the Yale Hunger and Homelessness Action Project.

Regina Bok, MS

Ms. Bok has been employed in leadership positions at Continuum of Care, Inc. for the past 18 years and has had more than 40 years of professional experience in residential care and support services for varied disenfranchised populations in Connecticut and California. She currently directs eleven (11) of Continuum's varied programs that support individuals with intellectual/developmental disabilities and autism with residential options, individualized day services and vocational programming.
Reggie has a respected statewide reputation as a dedicated client-focused advocate for individuals with exceptionally challenging diagnoses and behaviors. She holds a Master's Degree in Community Psychology and Human Service Administration. She is a member of the Autism Society of America, the American Association on Intellectual and Developmental Disabilities, the National Alliance for the Mentally Ill, the American Society on Aging and the Connecticut Association of Non-Profits.
Kathy DellaMura, RN, BSPA

Mrs. DellaMura has been a valued member of the Continuum Home Health family since its inception and has more than 12 years of experience in dealing with the special needs of the mentally ill. She is responsible for the clinical and operational oversight of Continuum Home Health, Inc., an agency that provides more than 100,000 behavioral health related visits per year. Mrs. DellaMura's background as a home care case manager, a wellness consultant, a nursing supervisor and as the director of a geriatric health care center helps ensure that Continuum Home Health's programs are tailored to patients' specific clinical needs.
Mrs. DellaMura's professional experience also includes leadership positions in inpatient medical, surgical, and critical care nursing. Additionally, she served for five years as an Assistant Director at a substance abuse facility. Mrs. DellaMura holds degrees in nursing and healthcare administration.
Monica O'Connor
Vice President, Facilities Management

Mrs. O'Connor has been employed at Continuum of Care, Inc. for over 7 years. She is responsible for the maintenance oversight of Continuum of Care's 28 residential and 2 commercial properties. She is the agency liaison for all development projects such as property searches, new construction and HUD developments. Mrs. O'Connor works collaboratively with multiple City and town departments such as Public Health, Livable Cities Initiative, and Fire Marshall Departments.
Mrs. O'Connor is responsible for the oversight of Continuum Facilities Management (CFM) work crew an innovative program that provides residents with employment opportunities and training while assisting the agency in maintaining its properties and/or programs. Under Mrs. O'Connor's management CFM has been able to expand its services to other local not-for-profits by providing landscaping and moving services.
