The Supported Living Program (SLP) affords individuals the opportunities to live in their own apartment with supports. The program is staffed 24 hours a day and provides residents with the opportunities to learn the daily life skills necessary to live independently.

The SLP provides on-site case management, counseling, arranges community outings, and organizes activities. Residents are assisted in cooking meals, navigating transportation, scheduling necessary medical appointments, and seeking employment.

Medication certified staff administers medication and when applicable, nursing/medical support, or outside clinical consult is obtained as needed.


The Supported Living Program provides supportive services in New Haven, Bridgeport, and Meriden. To be considered for placement, individuals must be eligible for DDS services. For further information contact:
